Performance Indicators
This web page provides a high level illustration of the progress that we`ve made in our Strategic Plan. These measures are responsible for helping us improve the health, safety, and well-being of Hoosiers across the state through the services we provide. With our values of integrity, innovation, collaboration, excellence, and dedication, OPHPM has worked with every reporting program area to make each measure reflective of the program`s work. The team placed special emphasis on how we can work towards achieving health equity and the role programs at the agency play in promoting optimal health among all populations.
The five goals for this Strategic Plan cycle include: Goal 1: Develop and strengthen strategic partnerships to improve public health
Goal 2: Promote and provide transparent public health data
Goal 3: Ensure the conditions for optimal health are available to all Hoosiers
Goal 4: Mitigate and prepare for public health threats
Goal 5: Improve organizational health and be an employer of choice
The Indiana State Department of Health steered the making of the 2018 - 2020 Strategic Plan to answer the question: What can ISDH do to improve the delivery of public health services to the citizens of Indiana? ISDH considered key support functions required for efficiency and effectiveness and sought to articulate what it plans to achieve as an organization, the actions it will take, and how it will measure success. The identified objectives and activities to follow are embedded within ISDH`s performance management system (VMSG) and will be tracked regularly throughout the life of this plan.
With ISDH`s efforts to continuously adapt and analyze our processes, OPHPM is working with program areas to edit their measures in preparation for updating our current Strategic Plan. The Strategic Plan is a living document that will be continuously evaluated and updated as public health evolves in Indiana to be reflective of the services the agency provides. Please take time to review the current strategic plan (found on the Intranet) and reflect on how your own program plays a role in providing services to the State of Indiana and how your program area could also track their progress.

2) Reduce racial and ethnic disparities in infant mortality
3) Increase the percentage of Hoosiers at a healthy weight
4) Reduce the burden of tobacco use in Indiana
5) Reduce morbidity and mortality from chronic disease
1) Address the opioid epidemic
2) Ensure Hoosiers are appropriately immunized
3) Identify children who have been exposed to lead
4) Ensure Indiana is ready to respond to public health emergencies
5) Expand HIV coordination and care delivery systems
6) Improve the continuation of care for Hepatitis B and C
